ABSTRACT

Abstract Background: Although electrical and structural remodeling has been recognized to be important in the pathophysiology of atrial fibrillation, the mechanisms underlying remodeling process are unknown. There has been increasing interest in the involvement of inflammatory molecules and adipokines released from the epicardial fat tissue in the pathophysiology of atrial fibrillation. Objectives: In our study, we aimed to investigate the relationship of atrial fibrillation with increased epicardial adipose tissue, inflammatory molecules released from this tissue and omentin. Methods: Thirty-six patients who were followed up with a diagnosis of permanent AF at the cardiology outpatient clinic 33 individuals without atrial fibrillation (controls) were included in the study. Epicardial adipose tissue thickness of patients was measured by echocardiography. Serum omentin, IL 6, IL 1 beta, TNF alpha and CRP levels were measured. Man-Whitney U test was performed for comparisons and significance was established at 5% (p<0.05). Results: Epicardial adipose tissue thickness was significantly greater in the patient group (6mm [4-5.5]) than controls (4mm [3-5.5]) (p <0.001). No significant difference was found in the concentrations of omentin or inflammatory molecules between the groups. Conclusion: No relationship was found between atrial fibrillation and serum levels or omentin or inflammatory markers. A relationship between epicardial adipose tissue thickness measured by echocardiography and atrial fibrillation was determined.

ABSTRACT

Objective:To investigate the relationship of von Willebrand factor cleaving enzyme(ADAMTS-13), ratio of mean platelet volume to platelet count (MPV/P), intelectin-1 (ITLN-1) and thrombolysis in myocardial infarction (TIMI) blood flow classification in acute myocardial infarction patients who performed percutaneous coronary intervention(PCI) and the value of predicting postoperative major cardiovascular adverse events (MACE).Methods:A total of 126 patients with acute myocardial infarction treated with PCI in Yuechi County People′s Hospital from January 2019 to January 2021 were selected and divided into MACE group (25 cases) and no MACE group (101 cases) according of the prognosis. The levels of ADAMTS-13, MPV/P and ITLN-1 were compared between the two groups, the correlation of above index and TIMI blood flow classification and the value of predicting postoperative MACE were analyzed. Results:The levels of ADAMTS-13 and ITLN-1 in the MACE group were lower than those in the MACE group, and the level of MPV/P was higher than that in the MACE group: (132.59 ± 43.05) μg/L vs. (186.33 ± 58.17) μg/L, (255.36 ± 80.24) mg/L vs. (342.65 ± 91.18) mg/L, (0.06 ± 0.02) fl/( × 10 9/L) vs. (0.04 ± 0.01) fl/( ×10 9/L), there were statistical differences ( P<0.05). The results of Spearman correlation analysis showed that the levels of ADAMTS-13 and ITLN-1 were positively correlated with TIMI blood flow classification ( r = 0.692, 0.622, P<0.01) , the level of MPV/P was negatively correlated with TIMI blood flow classification ( r = -0.665, P<0.01). The area under the curve of ADAMTS-13, MPV/P combined with ITLN-1 in protecting the MACE was 0.872. Conclusions:ADAMTS-13, MPV/P and ITLN-1 are related to the TIMI blood flow classification and MACE in patients with acute myocardial infarction after PCI, and the combined detection can be used as a reliable predictor of MACE.

ABSTRACT

OBJECTIVES: We aimed to evaluate the effect of insulin resistance (IR) on serum Intelectin-1 and endocrinological hormones levels in obese and non-obese women with and without polycystic ovary syndrome (PCOS) in Basrah, Iraq.METHODS: From 124 women volunteers, 60 patients with primary and 64 patients with secondary, while 56 normal ovulatory women were taken as controls. Their fasting insulin hormone, intelectin-1, anti-Mullerian hormone, luteinizing hormone (LH), follicle stimulating hormone (FSH), prolactin (PRL), estradiol (E2) and testosterones (T) were determined by ELISA methods. BMI, glucose and quantitative insulin sensitivity check index as well as IR was determined by the homeostasis model assessment.RESULTS: A significant changes (P<0.05) were seen in the level of homeostasis model assessment-IR, E2 and T. Levels of anti-Mullerian hormone, LH, LH/FSH ratio and prolactin were significantly (P<0.01) increased and level of intelectin-1 and E2/T ratio were significantly (P<0.01) decreased, while quantitative insulin sensitivity check index level was not significantly different (P>0.05) between the patients (1°PCOS and 2°PCOS) and control groups. On the other hand, our data reported that FSH level was significantly (P<0.05) lower in obese and higher in non-obese patients with PCOS as compared to control group.CONCLUSION: Levels of intelectin-1 and endocrinological hormones have significantly associated with body mass index, IR and physical activity in patients and normal groups and the strategies that can modulate levels of these parameters would improve metabolic disarrangements in women with PCOS.
